Gateway
Care Center provides around-the-clock long-term care for residents that require
more services than assisted living facilities can provide. Gateway Care Center’s
caring professional staff provide quality care, and families know their loved
ones will be in the best hands possible. Our long-term care services provide
around-the-clock nursing care in a homelike setting by some of the best-trained
registered and licensed nurses in the industry. Like all of our employees, our
nurses pledge to treat every resident with the utmost respect and dignity and
strive to help them function at the highest level possible. Our long-term care
services are provided when and if they are needed. Our goal is to make all
residents as comfortable as possible. At Gateway Care Center we are dedicated to providing
customized services to meet the individual needs of our residents. You can rest
assured that you and your loved one will receive the highest level of care and
services that you require.
THERAPY
We
provide a comprehensive rehabilitation program, which includes Physical,
Occupational and Speech Therapy services. Rehabilitation therapy is very
individualized, with the goal of assisting you to achieve and to maintain your
highest level of functionality. The therapy team could help you recover from
many conditions such as strokes or hip fractures. They also assist anyone who
may simply need to regain their strength after a lengthy illness. Our rehab
therapy staff works closely with all other departments to ensure continuity and
care.
We are Medicare and Medicaid Certified, and accept most private Insurances; also accredited by JCAHO.

Long term care (and Sub Acute Rehabilitation) organizations seek Joint Commission accreditation as a means to improve organization performance, as well as to demonstrate their commitment to quality and safety to external stakeholders. Current research shows that long term care accreditation is a risk reduction activity that reduces the likelihood of negative patient or resident outcomes and incrementally improves an
organization’s performance. In addition, accreditation enhances community
confidence, supports staff recruitment and education, and provides a structure
for organizing business and care processes. Accreditation also allows providers
to differentiate themselves in a market of more than 15,000 nursing facilities

Survey process
To earn and maintain accreditation, a long term care (and Sub Acute Rehab) organization must undergo an on-site survey by a Joint Commission surveyor(s) approximately every
three years. The objective of the survey is not only to evaluate the long term
care organization, but to provide education and guidance that will help staff
continue to improve its performance. The survey process evaluates actual care
processes by tracing patients and residents through the care, treatment and
services they receive and also analyzes key operational systems that directly
affect the quality and safety of patient or resident care.
Surveys are conducted by experienced long term care professionals who have at
least five years of leadership experience in a long term care facility; and a
strong educational background – all surveyors have master’s degrees and many
have doctorate degrees. Surveyors are required to complete and pass a
surveyor certification examination to help ensure the knowledge and
consistency of The Joint Commission’s surveyor cadre.
